1. Provide excellent care and treatment to patients in the comfort of their own homes as a Physical Therapist.
2. Join and work collaboratively with a dynamic Home Health team to deliver high-quality care.
3. Help patients regain their mobility and independence through effective physical therapy techniques.
4. Demonstrate dedication and a patient-centered approach in all interactions with patients.
5. Utilize clinical expertise and knowledge to assess patient needs and develop personalized treatment plans.
6. Monitor and document patient progress, adjusting treatment plans as needed.
7. Educate patients and their families on exercises and techniques to continue care at home.
8. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
9. Maintain accurate and timely records of patient care and progress.
10. Adhere to all ethical and legal standards in patient care and confidentiality.
11. Stay updated on industry advancements and best practices in physical therapy.
12. Contribute to a positive and supportive work environment.
13. Participate in team meetings and provide input and feedback on patient care.
14. Comply with all safety and infection control protocols.
