
Transition Nurse Navigator
The main goal of the Care Transition Nurse is readmission prevention and patient safety. The Care Transition Nurse will be responsible for making contact with patients (identified per eligibility criteria for the program) within 2472 hours after discharge to evaluate patient education, determine patients' compliance with discharge instructions, medications and follow up appointments. The Care Transition Nurse will continue to follow patient (telephonically) for 30 ~ 90 days post discharge to assist with post hospitalization transition and healthcare navigation. Identifies the needs of patients and families and coordinates internal and external community resources within the first month of hospital discharge to reduce posthospitalization illness/disease and decrease readmissions. Supports discharge instructions on AVS, and validates all services, equipment are in place after discharge.
Earn 10.0 continuing education credits annually
Minimum 4,000 hours of RN experience
Minimum two years of clinical experience in acute or ambulatory care
Ability to respond quickly to patient needs and work at fast pace
