- Conduct case management activities: The Analyst will be responsible for conducting case management activities, including assessing member needs, developing care plans, coordinating services, and monitoring progress.
- Ensure quality care delivery: The Analyst will play a critical role in ensuring the delivery of quality care to Aetna members by closely monitoring and evaluating the effectiveness of care plans and services.
- Collaborate with healthcare professionals: This role will require the Analyst to collaborate with healthcare professionals, including physicians, nurses, and social workers, to develop and implement effective care plans for Aetna members.
- Provide support to members: The Analyst will be responsible for providing support and guidance to Aetna members, helping them understand their healthcare options and connecting them with appropriate resources.
- Maintain accurate records: The Analyst will be responsible for maintaining accurate and up-to-date records of member interactions, care plans, and services provided, in accordance with company and regulatory standards.

Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting.
Bachelor's Degree In A Relevant Field Such As Healthcare Administration, Social Work, Or Public Health.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Clients, Providers, And Other Team Members.
Knowledge Of Local Resources And Community-Based Services In Macon, Moultrie, Piatt, Or Champaign Counties, Il.
Ability To Work Independently And Manage A Diverse Caseload, While Adhering To Company Policies And Procedures.
