Job Description

  1. Conduct analysis and provide insights on case management processes and operations.

  13. Participate in meetings and discussions to provide insights and recommendations on case management operations.

  15. Maintain accurate and detailed records of case management activities and outcomes.

Job Qualifications
  • Bachelor's Degree In A Related Field, Such As Healthcare Administration, Social Work, Or Business Administration.

  • Previous Experience In Case Management, Preferably In A Healthcare Setting.

  • Knowledge Of St. Clair County, Il And Surrounding Areas, Including Resources And Community Organizations.

  • Strong Analytical And Problem-Solving Skills, With The Ability To Assess And Prioritize Complex Cases.

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Relationships And Collaborate With A Diverse Group Of Stakeholders.
