  1. Develop and implement population health management strategies to improve the health outcomes of Aetna members.
  2. Collaborate with internal and external stakeholders to identify and address healthcare disparities and promote health equity.
  3. Utilize evidence-based practices and data to inform decision-making and measure the effectiveness of population health programs.
  4. Provide leadership and guidance to a team of population health professionals, including setting goals, tracking progress, and promoting a collaborative work environment.
  5. Conduct ongoing research and analysis to identify areas for improvement and develop solutions to address population health challenges.
  6. Stay up-to-date on industry trends and best practices in population health management and integrate them into Aetna's strategies.
  7. Collaborate with other departments within Aetna, such as clinical operations and data analytics, to align population health initiatives with overall company goals.
  8. Act as a liaison between Aetna and community organizations to build partnerships and support community-based population health initiatives.
  9. Develop and manage budgets for population health programs and ensure cost-effective solutions are implemented.
  10. Continuously monitor and evaluate the effectiveness of population health programs and make recommendations for improvement.
  11. Communicate population health initiatives and progress to senior leadership and other stakeholders.
  12. Foster a culture of diversity, equity, and inclusion within the population health team and across the organization.
  13. Adhere to all company policies, procedures, and regulatory requirements related to population health management.
  14. Act as a champion for Aetna's mission to build a healthier world, one person at a time.

Job Qualifications
  • Bachelor's Degree In Public Health, Healthcare Administration, Or A Related Field.

  • Minimum Of 5 Years Of Experience In Population Health Management, Disease Management, Or Care Coordination.

  • Strong Understanding Of Healthcare Systems, Policies, And Regulations.

  • Experience With Data Analysis And Reporting, Including Proficiency In Microsoft Excel And Other Analytical Tools.

  • Excellent Communication And Leadership Skills, With The Ability To Collaborate With Diverse Teams And Effectively Communicate With Stakeholders At All Levels.
