  1. Develop and implement population health strategies and initiatives aimed at improving the health and well-being of communities.
  2. Collaborate with cross-functional teams to identify and address population health needs and gaps.
  3. Conduct data analysis and utilize data to inform decision-making and measure the effectiveness of population health programs.
  4. Lead and manage a team of population health professionals, providing direction, support, and mentorship.
  5. Establish and maintain relationships with community stakeholders, including healthcare providers, local organizations, and government agencies.
  6. Stay current on industry trends and best practices in population health management.
  7. Develop and manage budgets for population health programs and initiatives.
  8. Communicate with internal and external stakeholders about the goals, progress, and impact of population health efforts.
  9. Ensure compliance with all relevant laws, regulations, and policies related to population health.
  10. Continuously evaluate and adjust population health strategies to optimize outcomes and address emerging needs.
  11. Act as a subject matter expert on population health principles and practices, providing guidance and support to colleagues and team members.
  12. Foster a culture of innovation, collaboration, and continuous improvement within the population health team and across the organization.
  13. Represent the organization at conferences, meetings, and other events related to population health.
  14. Develop and maintain partnerships with community organizations and resources to support population health efforts.
  15. Contribute to the development of long-term population health goals and objectives for the organization.

Job Qualifications
  • A Master's Degree In Public Health, Healthcare Administration, Or A Related Field.

  • At Least 5 Years Of Experience In A Population Health Management Role, Preferably In A Healthcare Setting.

  • Knowledge Of Healthcare Data Analytics And Ability To Use Data To Drive Population Health Initiatives.

  • Strong Leadership Skills And Experience Managing A Team Or Department.

  • Familiarity With Healthcare Regulations And Policies, Including Hipaa And Affordable Care Act, And Ability To Ensure Compliance Within Population Health Programs.
