  1. Provide advanced clinical expertise and leadership in medication management to ensure the safe and effective use of medications for patients.
  2. Collaborate with healthcare providers to develop and implement comprehensive medication therapy plans for patients based on their individual needs.
  3. Serve as a resource and mentor for other pharmacists and healthcare professionals, sharing knowledge and best practices to improve patient care.
  4. Conduct medication reviews and provide drug therapy recommendations to optimize patient outcomes and minimize potential adverse effects.
  5. Participate in patient rounds and interdisciplinary team meetings to provide input on medication-related issues and help develop treatment plans.
  6. Stay current with new medications, treatment guidelines, and advancements in the pharmacy field to continuously improve patient care.
  7. Maintain accurate and up-to-date patient records, including medication histories and drug interactions, to ensure safe and effective medication use.
  8. Oversee the work of pharmacy technicians and support staff to ensure accurate and efficient dispensing of medications.
  9. Provide patient education and counseling on medication use, including potential side effects and drug interactions.
  10. Adhere to all state and federal laws and regulations related to medication dispensing and record keeping.
  11. Serve as a liaison between the pharmacy and other healthcare departments, fostering effective communication and collaboration for the benefit of patients.
  12. Continuously evaluate and improve pharmacy services and processes to enhance patient satisfaction and safety.
  13. Act as a role model for professionalism, integrity, and ethical behavior in all interactions with patients, colleagues, and other healthcare professionals.
  14. Participate in quality improvement initiatives and research projects to contribute to the advancement of pharmacy practice.
  15. Represent Allina Health System in a positive and professional manner at all times, promoting the organization's mission and values.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School

  • Current And Active Pharmacy License In The State Of Minnesota

  • Board Certification In A Specialty Area (E.g. Ambulatory Care, Critical Care, Oncology)

  • Minimum Of 5 Years Of Experience As A Clinical Pharmacist, Preferably In A Hospital Or Health System Setting

  • Strong Knowledge Of Medication Therapy Management, Clinical Guidelines, And Drug Information Resources.
