Amedisys is seeking a dedicated Care Transitions Coordinator to join our team and help us achieve our mission of improving the health and well-being of our patients. As a Care Transitions Coordinator, you will play a critical role in ensuring a smooth and effective transfer of care for our patients, while also building and maintaining relationships with healthcare providers and community resources.   1. Coordinate and facilitate the transition of care for patients between healthcare settings, including hospitals, skilled nursing facilities, and home health agencies.
  2. Develop and maintain relationships with healthcare providers and community resources to ensure timely and effective care transitions for patients.
  3. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
  4. Conduct comprehensive assessments to identify patient needs and coordinate appropriate services.
  5. Communicate and educate patients and their families about the transition of care process and provide necessary support and resources.
  6. Monitor and track patient progress and outcomes during and after the transition of care.
  7. Collaborate with insurance companies and other payers to ensure proper coverage and reimbursement for services.
  8. Stay up-to-date on industry trends and best practices related to care transitions and care coordination.
  9. Comply with all regulatory and organizational policies and procedures.
  10. Maintain accurate and timely documentation of all care coordination activities.
  11. Participate in interdisciplinary team meetings and provide insights and recommendations for improving care transitions.
  12. Serve as an advocate for patients, ensuring their needs and preferences are met throughout the transition of care process.
  13. Maintain confidentiality and adhere to ethical standards in all interactions.
  14. Continuously strive to improve and streamline care transition processes to enhance the patient experience.
  15. Collaborate with the marketing team to promote Amedisys and its services to potential referral sources.
  16. Provide support and guidance to colleagues and new team members as needed.

Job Qualifications
  • Excellent Communication And Interpersonal Skills.

  • Bachelor's Degree In Nursing, Social Work, Or Related Field.

  • Minimum Of 2 Years Experience In Healthcare Or Case Management.

  • Strong Understanding Of Care Transitions And Discharge Planning Processes.

  • Experience Working With Diverse Patient Populations And Navigating Complex Healthcare Systems.
