
LPN Clinical Patient Services Coordinator

- Serve as the primary point of contact for patients, caregivers, and healthcare professionals to coordinate and manage patient care services.
- Communicate and collaborate with healthcare team members to ensure seamless patient care and treatment plans.
- Schedule patient appointments and coordinate transportation as needed.
- Maintain accurate and up-to-date patient records and documentation.
- Monitor patient progress and communicate any changes or concerns to the healthcare team.
- Provide education and support to patients and their families regarding their care plans.
- Act as a liaison between patients and insurance providers to ensure coverage for necessary services.
- Assist with coordinating referrals and obtaining necessary authorizations for patient services.
- Monitor and track medical supply inventory and order supplies as needed.
- Participate in quality improvement initiatives to ensure the highest level of patient care.
- Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
- Stay current on industry trends and best practices to continually improve the patient experience.
- Collaborate with other departments and team members to ensure smooth operations and exceptional patient care.
- Participate in training and development opportunities to enhance knowledge and skills.
- Uphold the mission, vision, and values of the organization in all interactions with patients, caregivers, and team members.
Active Lpn License: The Candidate Must Hold An Active And Unrestricted Lpn License In The State Where The Position Is Located.
Clinical Experience: A Minimum Of 2-3 Years Of Clinical Experience As An Lpn, Preferably In A Home Health Or Hospice Setting.
Patient Care Coordination Skills: The Candidate Should Have A Strong Understanding Of Patient Care Coordination, Including Scheduling, Care Planning, And Coordinating With Other Healthcare Professionals.
Familiarity With Electronic Health Records: Amedisys Utilizes Electronic Health Records, So The Ideal Candidate Should Have Experience And Proficiency With This Technology.
Communication And Leadership Skills: This Role Requires Strong Communication Skills, As Well As The Ability To Lead And Work Collaboratively With A Team Of Healthcare Professionals. Previous Experience In A Leadership Or Management Role Is Preferred.
