  1. Serve as the primary point of contact for patients, caregivers, and healthcare professionals to coordinate and manage patient care services.
  2. Communicate and collaborate with healthcare team members to ensure seamless patient care and treatment plans.
  3. Schedule patient appointments and coordinate transportation as needed.
  4. Maintain accurate and up-to-date patient records and documentation.
  5. Monitor patient progress and communicate any changes or concerns to the healthcare team.
  6. Provide education and support to patients and their families regarding their care plans.
  7. Act as a liaison between patients and insurance providers to ensure coverage for necessary services.
  8. Assist with coordinating referrals and obtaining necessary authorizations for patient services.
  9. Monitor and track medical supply inventory and order supplies as needed.
  10. Participate in quality improvement initiatives to ensure the highest level of patient care.
  11. Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
  12. Stay current on industry trends and best practices to continually improve the patient experience.
  13. Collaborate with other departments and team members to ensure smooth operations and exceptional patient care.
  14. Participate in training and development opportunities to enhance knowledge and skills.
  15. Uphold the mission, vision, and values of the organization in all interactions with patients, caregivers, and team members.

Job Qualifications
  • Active Lpn License: The Candidate Must Hold An Active And Unrestricted Lpn License In The State Where The Position Is Located.

  • Clinical Experience: A Minimum Of 2-3 Years Of Clinical Experience As An Lpn, Preferably In A Home Health Or Hospice Setting.

  • Patient Care Coordination Skills: The Candidate Should Have A Strong Understanding Of Patient Care Coordination, Including Scheduling, Care Planning, And Coordinating With Other Healthcare Professionals.

  • Familiarity With Electronic Health Records: Amedisys Utilizes Electronic Health Records, So The Ideal Candidate Should Have Experience And Proficiency With This Technology.

  • Communication And Leadership Skills: This Role Requires Strong Communication Skills, As Well As The Ability To Lead And Work Collaboratively With A Team Of Healthcare Professionals. Previous Experience In A Leadership Or Management Role Is Preferred.
