1. Provide high-quality nursing care to patients in their homes.
2. Assess patient’s health status and develop individualized care plans.
3. Administer medications and treatments as prescribed by the physician.
4. Monitor and record patient’s vital signs and medical history.
5. Collaborate with physicians and other healthcare professionals to ensure coordinated and comprehensive care.
6. Educate patients and their families on their health conditions and self-care techniques.
7. Communicate effectively with patients, families, and caregivers to establish trust and promote patient satisfaction.
8. Maintain accurate and timely documentation of all patient care activities.
9. Adhere to all legal, ethical, and professional standards in healthcare.
10. Participate in ongoing education and training programs to continuously improve clinical skills and knowledge.
11. Follow infection control, safety, and emergency protocols to maintain a safe environment for patients and staff.
12. Demonstrate empathy, compassion, and respect towards patients and their families.
13. Act as a liaison between patients, families, and healthcare providers.
14. Maintain confidentiality of patient information and adhere to HIPAA regulations.
15. Maintain a positive and professional attitude at all times.
16. Collaborate with interdisciplinary team members to provide comprehensive and holistic care.
17. Continuously evaluate and monitor patient progress and adjust care plans accordingly.
18. Report any changes in patient’s condition or concerns to the appropriate healthcare provider.
19. Attend team meetings and contribute to the development of patient care plans.
