  1. Conduct comprehensive assessments of patients' social and emotional needs.
  2. Develop individualized care plans in collaboration with patients, families, and healthcare team.
  3. Provide counseling and support to patients and families to address psychosocial issues related to illness and treatment.
  4. Advocate for patients and connect them with community resources to address their social and emotional needs.
  5. Collaborate with interdisciplinary team to coordinate and integrate patient care.
  6. Monitor and evaluate patients' progress and adjust care plans accordingly.
  7. Maintain accurate and timely documentation of patient interactions and interventions.
  8. Educate patients and families about available resources and support services.
  9. Participate in team meetings and contribute to the development of patient care plans.
  10. Stay up-to-date with best practices and current trends in social work and healthcare.
  11. Adhere to all ethical and professional standards in social work practice.
  12. Maintain confidentiality and respect patients' privacy.
  13. Communicate effectively and professionally with patients, families, and healthcare team.
  14. Collaborate with other social workers and community agencies to provide comprehensive care to patients.
  15. Help improve the overall well-being of patients by addressing their social and emotional needs.

Job Qualifications
  • State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw).

  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In Hospice Or Home Health.

  • Knowledge And Understanding Of Medical Terminology, Disease Processes, And Psychosocial Issues Related To Chronic And Terminal Illnesses.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With A Multidisciplinary Team And Provide Support To Patients And Their Families During Difficult Times.
