1. Conduct initial assessments of patients' health status and develop personalized care plans in collaboration with the patient's physician and interdisciplinary team.
2. Coordinate and provide skilled nursing care to patients in the comfort of their own homes.
3. Educate patients and their families on disease management, medication administration, and other aspects of home health care.
4. Monitor and document patients' progress, symptoms, and any changes in their condition.
5. Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care for patients.
6. Communicate effectively and compassionately with patients and their families to address their concerns and provide emotional support.
7. Ensure compliance with all federal, state, and local regulations and Amedisys policies and procedures.
8. Participate in on-call rotation and provide after-hours care as needed.
9. Assist with the orientation and training of new staff members.
10. Maintain accurate and timely documentation of all patient care activities.
11. Stay current with best practices and developments in home health care and nursing through ongoing education and training.
12. Adhere to ethical and professional standards of nursing practice.
13. Maintain a safe and clean work environment, following infection control and safety protocols.
14. Represent Amedisys in a professional and positive manner at all times.
