- Coordinate and manage the care of patients in accordance with Amedisys standards and policies.
- Develop and implement individualized patient care plans based on comprehensive assessments.
- Monitor and evaluate patient progress and adjust care plans as needed.

- Conduct regular home visits to provide skilled nursing care and education to patients and their families.

Bachelor's Degree In Nursing Or Related Field: Amedisys Requires All Rn Case Managers To Have A Minimum Of A Bachelor's Degree In Nursing Or A Related Field. This Educational Background Provides A Strong Foundation In Nursing Principles And Prepares Individuals For The Diverse Responsibilities Of A Case Manager.
Active Rn License: Amedisys Requires All Rn Case Managers To Hold An Active Rn License In The State Where They Will Be Practicing. This Ensures That The Individual Has Met The Necessary Requirements To Practice Nursing, Including Passing The Nclex Exam.
Minimum Of 2 Years Of Nursing Experience: Amedisys Prefers Candidates For The Rn Case Manager Position To Have A Minimum Of 2 Years Of Clinical Nursing Experience. This Experience Provides A Solid Understanding Of Patient Care And Allows The Individual To Effectively Coordinate And Manage Care For Patients.
Case Management Certification: While Not Always Required, Amedisys Prefers Rn Case Managers To Have A Certification In Case Management, Such As The Certified Case Manager (Ccm) Or Case Management Administrator (Cmac) Certification.
