Job Description

As a member of our team, you will have the opportunity to use your nursing expertise to provide comprehensive care to patients and their families, while also collaborating with an interdisciplinary team to ensure the best possible outcomes.   1. Provide compassionate and high-quality care to patients in the Hospice program.
  2. Utilize nursing expertise to assess, plan, implement, and evaluate patient care.
  3. Collaborate with an interdisciplinary team to develop and implement individualized care plans for patients.
  4. Coordinate and communicate with physicians, other healthcare providers, and family members to ensure continuity of care.
  5. Monitor and document patient progress, identifying and addressing any changes in condition.
  6. Administer medications and treatments as prescribed by the physician.
  7. Educate and support patients and families on end-of-life care, including pain management and symptom control.
  8. Ensure compliance with all regulatory and accreditation standards.
  9. Maintain accurate and timely documentation of all patient care activities.
  10. Participate in quality improvement initiatives to improve patient outcomes and promote best practices.
  11. Provide emotional support and counseling to patients and families during a difficult time.
  12. Serve as an advocate for patients and their families, ensuring their needs and wishes are met.
  13. Participate in on-call rotation and respond to patient needs outside of regular business hours.
  14. Use excellent communication skills to effectively communicate with patients, families, and colleagues.
  15. Adhere to Amedisys' values and ethical standards, promoting a positive and supportive work environment.
