Job Description

At Ascension Health, we are looking for a Director of Case Management to join our team and help us provide quality, compassionate care to our patients. We are a faith-based healthcare organization and strongly believe that every person we serve should be treated with dignity and respect.The ideal candidate for this role will have a passion for providing excellent patient care and be highly organized, detail-oriented, and have excellent interpersonal skills. We are looking for a leader who is committed to developing an effective case management program that will improve patient outcomes, reduce costs, and create a seamless experience for our patients.To be successful in this role, applicants must have a minimum of a Bachelor's degree in Nursing and at least five years of clinical and administrative experience in a healthcare, case management setting. They should also have proven experience leading a case management team, developing and/or implementing case management process improvements, and have a proven track record of meeting goals and objectives. In addition, the successful candidate must have knowledge of regulatory and accreditation standards related to case management, as well as excellent communication and problem-solving skills.

Responsibilities for Director of Case Management:

  1. Develop and implement a comprehensive case management program that will improve patient outcomes, reduce costs, and create a seamless experience for our patients.
  2. Lead and manage a case management team, providing guidance and direction to ensure optimal performance.
  3. Ensure compliance with all regulatory and accreditation standards related to case management.
  4. Monitor and review case management processes and procedures, making recommendations for process improvements.
  5. Develop and monitor goals and objectives to ensure that the case management team meets standards of quality and efficiency.
  6. Establish and maintain relationships with internal and external stakeholders to ensure optimal communication and patient care.
  7. Monitor and analyze patient data to identify trends and opportunities for improvement.
  8. Develop and implement patient-centered care plans that are tailored to meet the individual needs of each patient.
  9. Provide education and guidance to new and existing staff members on case management policies and procedures.
  10. Collaborate with other departments and healthcare providers to ensure seamless patient care.
