3. Conduct outreach and engage with community members to raise awareness of available health services and programs.
4. Collaborate with other healthcare professionals and community organizations to develop and implement health education and prevention programs.
5. Provide support and guidance to community members in navigating the healthcare system, including scheduling appointments, completing paperwork, and understanding insurance coverage.
6. Maintain accurate and up-to-date records of community outreach activities and referrals made.
7. Serve as an advocate for community members, ensuring their voices and needs are heard and addressed by healthcare providers and organizations.
8. Participate in community events and initiatives to promote Atrium Health's mission and services.
9. Continuously evaluate and assess the effectiveness of community health programs and make recommendations for improvement.
10. Stay informed about current health trends, policies, and resources to inform and enhance community outreach efforts.
11. Uphold Atrium Health's values of compassion, respect, integrity, teamwork, and excellence in all interactions with community members and colleagues.
12. Adhere to all legal and ethical standards, as well as Atrium Health's policies and procedures, in the performance of duties.
