
Medication Reconciliation Technician

- Review and verify patient medication lists to ensure accuracy and safety.
- Identify and address any discrepancies or potential issues with patient medications.

- Maintain confidentiality and adhere to all HIPAA regulations.

Completion Of An Accredited Pharmacy Technician Program Or Equivalent Education And Experience.
Current Certification As A Pharmacy Technician (Cpht) From The Pharmacy Technician Certification Board (Ptcb).
Minimum Of 1 Year Experience In A Healthcare Setting, Preferably In A Pharmacy Or Medication Reconciliation Role.
Knowledge Of Medication Names, Dosages, And Routes Of Administration.
Ability To Accurately And Efficiently Enter And Retrieve Data In Electronic Health Record Systems.
