Job Description

Atrium Health is seeking an energetic and experienced Solid Organ Transplant Pharmacist- Pediatrics to join our team. In this role, you will provide comprehensive pharmaceutical care for pediatric patients before, during, and after solid organ transplantation. You will be responsible for the clinical management of medications throughout the transplant process, including medication reconciliation, medication order review, and drug information. The ideal candidate for this position must possess excellent patient care skills, professional communication abilities, and a commitment to providing the highest level of care for pediatric patients. Responsibilities:

  1. Provide comprehensive pharmaceutical care for pediatric patients before, during, and after solid organ transplantation.
  2. Perform medication reconciliation, medication order review, and drug information.
  3. Build strong relationships with patients and their families, providing emotional and psychological support.
  4. Monitor and manage patients’ medication plans to ensure compliance and safety.
  5. Ensure accuracy of medication orders and provide timely follow-up with providers and other staff.
  6. Educate patients and their families on the importance of medication adherence.
  7. Communicate effectively with other healthcare professionals to ensure seamless care for pediatric patients.
  8. Participate in research and evaluation activities related to solid organ transplantation.
  9. Stay abreast of relevant clinical and technical advancements in pediatric solid organ transplantation.

Job Qualifications
  • Ability To Work In A Fast-Paced Environment

  • Board Certification In Pharmacotherapy
