Medical Social Worker

At BAYADA Home Health Care, we are looking for a Medical Social Worker who can provide emotional support to patients and their families and ensure their physical, social, and emotional needs are met. We are seeking an experienced, compassionate, and highly organized individual who is passionate about helping others and making a positive impact on our clients’ lives.The successful candidate must possess a Master's Degree in Social Work from an accredited university, be licensed as a Medical Social Worker in the state in which they work, and have a minimum of two years of experience in a home health care setting. They must also be a team-player with excellent communication skills, the ability to multitask, and a strong commitment to providing quality care and services to our clients.

Responsibilities:

1. Provide emotional support and counseling to patients and their families.
2. Assess physical, social, and emotional needs of patients and develop treatment plans accordingly.
3. Provide psychosocial assessments and develop treatment plans for patients and their families.
4. Establish and maintain relationships with external care providers and other medical professionals.
5. Monitor and evaluate patient progress.
6. Collaborate with other home health care professionals to ensure quality care and services.
7. Develop and implement patient education programs.
8. Maintain accurate documentation of patient records and progress.
9. Communicate with patients, families, and other healthcare providers in a professional and compassionate manner.
10. Participate in team meetings and professional development activities.
