- Provide administrative support to case managers in coordinating patient care and ensuring quality outcomes.
- Assist with the development and implementation of care plans for patients.
- Communicate effectively with patients, families, and healthcare team members to gather and share necessary information.
- Maintain accurate and up-to-date patient records and documentation.
- Collaborate with case managers to identify and address patient needs and concerns.
- Schedule and coordinate appointments, referrals, and follow-up care for patients.
- Monitor and track patient progress and update case managers accordingly.
- Serve as a liaison between patients, families, and healthcare providers to ensure smooth and efficient care coordination.
- Provide emotional support and guidance to patients and families during difficult times.
- Adhere to all confidentiality and HIPAA regulations in handling patient information.
- Participate in team meetings and provide input on patient care plans and improvements.
- Continuously strive to improve processes and enhance the overall patient experience.
- Attend training and development programs to enhance knowledge and skills.
- Maintain a positive and professional attitude while representing the organization.
- Perform other duties and responsibilities as assigned by the case management team or supervisor.
High School Diploma Or Equivalent: A Minimum Of A High School Diploma Or Equivalent Is Typically Required For A Case Management Assistant Position At Baycare Health System. This Demonstrates A Basic Level Of Education And Readiness To Handle Administrative Tasks.
Previous Healthcare Experience: Previous Experience Working In A Healthcare Setting, Such As A Hospital Or Clinic, Is Often Preferred For This Role. This Experience Can Include Working As A Medical Assistant, Nursing Assistant, Or In A Similar Administrative Role.
Knowledge Of Medical Terminology: A Strong Understanding Of Medical Terminology Is Essential For A Case Management Assistant At Baycare Health System. This Includes A Familiarity With Common Medical Procedures, Conditions, And Medications.
Excellent Communication Skills: As A Case Management Assistant, You Will Be Communicating With Patients, Healthcare Professionals, And Insurance Companies On A Daily Basis. Therefore, Strong Communication Skills, Both Written And Verbal, Are Highly Valued For This Position.
