Nurse Case Manager

In this role, you will be responsible for providing case management services to members with complex medical needs. You will serve as a liaison between members, healthcare providers, and internal departments. To be successful in this role, you should have a Bachelor's degree in Nursing or a related field. You should be an experienced and registered nurse with a valid South Carolina license. Responsibilities:

1. Provide case management services to members with complex medical needs.
2. Serve as a liaison between members, healthcare providers, and internal departments.
3. Develop and implement individualized care plans for members with complex medical needs.
4. Monitor members’ progress and adjust care plans as needed.
5. Stay up-to-date with changes in healthcare laws and regulations.
6. Maintain records of case management activities.
7. Prepare reports for healthcare providers, internal departments, and other stakeholders.
8. Educate members and their families about health management and treatment options.
9. Advocate on behalf of members to ensure they receive appropriate and timely care.
10. Attend meetings and conferences related to case management.
