1. Manage, plan, and coordinate members' health care needs and services.

3. Provide quality care and support to members.

5. Develop and implement individualized care plans for members.
6. Monitor members’ health and progress on care plans.
7. Build and maintain relationships with members and their families.
8. Participate in outreach and educational activities.
9. Facilitate referrals to and coordinate with community resources.
10. Ensure compliance with legal, ethical, and regulatory standards.
