Intake Coordinator

The Intake Coordinator will be responsible for coordinating the intake process for hospice and home health services. The successful candidate will be able to assess patient eligibility and review medical records to determine the most appropriate level of care for each individual.To be considered for this role, you must have a minimum of two years of experience in a healthcare setting, preferably in hospice or home health. You must be knowledgeable in the assessment of patient eligibility and in developing care plans.
