Job Description

At Bridge Home Healthcare & Hospice, we strive to provide the highest quality of care for our patients and their families. We are looking for an experienced Intake Coordinator to join our team of compassionate and dedicated professionals. The Intake Coordinator will be responsible for coordinating the intake process for hospice and home health services. The successful candidate will be able to assess patient eligibility and review medical records to determine the most appropriate level of care for each individual.To be considered for this role, you must have a minimum of two years of experience in a healthcare setting, preferably in hospice or home health. You must be knowledgeable in the assessment of patient eligibility and in developing care plans. You must also have excellent interpersonal skills and be comfortable working with a variety of patients and families.If you are passionate about providing quality care and have the skills and experience required for this role, we look forward to receiving your application.

Responsibilities:

  1. Coordinate the intake process for hospice and home health services.
  2. Assess patient eligibility and review medical records to determine the most appropriate level of care for each individual.
  3. Develop care plans based on patient eligibility and medical records.
  4. Build strong relationships with patients and families.
  5. Answer questions and provide information to patients and families about hospice and home health services.
  6. Maintain accurate records of patient care and services provided.
  7. Monitor patient progress and adjust care plans accordingly.
  8. Collaborate with other healthcare professionals to ensure quality care.
  9. Remain up-to-date on all applicable laws and regulations related to hospice and home health services.
