
Care Coordination Assistant
Catholic Health Initiatives is seeking an experienced and passionate Care Coordination Assistant to join our team. In this role, you will be responsible for providing a variety of administrative and clinical services to support the care coordination team. We are looking for someone who is organized, detail-oriented, and able to work in a fast-paced environment. The ideal candidate will have experience in the healthcare field, excellent communication and problem-solving skills, and a commitment to providing excellent patient care.The successful candidate must have a minimum of an associate's degree in a healthcare-related field. At least two years of experience providing administrative and clinical support in a healthcare setting is also required. Knowledge of electronic medical records and healthcare processes is essential. Responsibilities:
- Provide administrative and clinical support to the care coordination team.
- Maintain accurate and organized records of patient care.
- Manage patient scheduling and appointment reminders.
- Communicate effectively with patients and other healthcare providers.
- Utilize electronic medical records and other healthcare software.
- Ensure compliance with healthcare regulations and procedures.
- Monitor patient care progress and document findings.
- Assist with billing and insurance processes.
- Provide patient education and support.
- Assist with patient transportation or other needs as necessary.
