Discharge Planner

Cedars-Sinai Medical Center is seeking an experienced and compassionate Discharge Planner to join our team. This individual will be responsible for facilitating the transition of patients from hospital stay to home or other care setting. The Discharge Planner will provide a comprehensive assessment of patient's needs and provide referrals and resources to ensure safe and appropriate discharge.The ideal candidate will have a Bachelor’s degree or equivalent in the fields of Nursing, Social Work, or Health Administration, and a minimum of two years' experience in a clinical setting or social services. This individual should have excellent communication skills, an interest in patient advocacy, and the ability to provide effective case management services. Additionally, the successful candidate should be organized, detail-oriented, and possess strong problem-solving skills.
