Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.
Active Pharmacy License In The State Of California.
Board Certification In Ambulatory Care Pharmacy Or Pharmacotherapy.
Minimum Of 2 Years Of Experience In A Pediatric Or Ambulatory Care Pharmacy Setting.
Strong Knowledge Of Medication Therapy Management, Medication Reconciliation, And Discharge Planning Processes.
