  1. Ensure safe and effective transition of pediatric patients from hospital to home.
  2. Provide exceptional pharmaceutical care to pediatric patients and their families.
  3. Collaborate with healthcare team to develop and implement medication therapy plans for pediatric patients.
  4. Monitor and assess medication regimens for appropriateness, safety, and efficacy.
  5. Educate patients and their families on proper medication use and potential side effects.
  6. Review and verify medication orders for accuracy and appropriateness.
  7. Participate in medication reconciliation activities to ensure continuity of care.
  8. Collaborate with other healthcare professionals to optimize patient outcomes.
  9. Stay up-to-date on current pediatric pharmacy practices and guidelines.
  10. Serve as a resource for pharmacy staff and healthcare team on pediatric medication-related issues.
  11. Document and maintain accurate patient records and medication profiles.
  12. Participate in quality improvement initiatives to enhance patient care.
  13. Adhere to all regulatory and safety standards.
  14. Communicate effectively with patients, families, and healthcare team members.
  15. Maintain confidentiality of patient information.
  16. Mentor and train pharmacy students and residents.
  17. Actively participate in departmental meetings and committees.
  18. Continuously evaluate and improve pharmacy processes to enhance patient care.
  19. Collaborate with insurance providers to ensure appropriate coverage for pediatric medications.
  20. Adhere to hospital policies and procedures.
  21. Provide excellent customer service to patients and families.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.

  • Active Pharmacy License In The State Of California.

  • Board Certification In Ambulatory Care Pharmacy Or Pharmacotherapy.

  • Minimum Of 2 Years Of Experience In A Pediatric Or Ambulatory Care Pharmacy Setting.

  • Strong Knowledge Of Medication Therapy Management, Medication Reconciliation, And Discharge Planning Processes.
