Behavioral Health Care Coordination Specialist

They should have strong experience in the areas of case management, care coordination, patient/family education, crisis intervention, and linkages to community resources. Responsibilities:

1. Develop comprehensive behavioral health care plans for patients and families.
2. Provide case management, care coordination, patient/family education, crisis intervention, and linkages to community resources.
3. Ensure that patient and family goals are being met and that services are appropriate and timely.
4. Utilize various modalities to provide patient/family education and resources to support behavioral health care coordination.
5. Coordinate with other healthcare professionals to ensure quality care is being provided.
6. Develop and maintain relationships with partners and stakeholders that provide services for our patients and families.
7. Maintain accurate records and documentation of behavioral health care coordination services.
8. Monitor patient progress and provide follow-up services as necessary.
9. Provide guidance and support to patients and families in order to promote healthy lifestyle choices and positive behavioral change.
10. Participate in collaborative meetings and initiatives with members of the healthcare team.
