Social Worker Care Manager

At Cincinnati Children's, we are looking for a compassionate and experienced Social Worker Care Manager to join our team. Our ideal candidate will be a patient advocate with the ability to connect with families, build meaningful relationships, and provide comprehensive support services. The Social Worker Care Manager will be responsible for providing care management, case management, and discharge planning services to assigned patients and their families. The successful candidate will act as a liaison between patients, families, and healthcare providers, working to ensure a successful transition between services. To be successful in this role, the Social Worker Care Manager will need to have excellent communication, problem-solving, and organizational skills. The candidate should also be comfortable working with a variety of stakeholders and have a strong commitment to providing patient-centered care. We are looking for a candidate who has a Master's degree in Social Work (MSW) from an accredited university, with at least two years of experience in a healthcare setting.
