1. Provide comprehensive social work services to patients and families undergoing kidney transplant, including psychosocial assessments, counseling, crisis intervention, and care coordination.
2. Collaborate with the multidisciplinary transplant team to develop and implement individualized care plans that address the psychosocial needs of patients and families.
3. Advocate for the well-being and rights of patients and families, ensuring their needs are met and their voices are heard.
4. Facilitate communication and collaboration between patients, families, healthcare providers, and community resources.
5. Provide education and support to patients and families regarding the transplant process, including potential challenges and coping strategies.
6. Conduct home visits and community outreach to assess and address any social or environmental barriers to care.
7. Maintain accurate and up-to-date documentation of all patient interactions and care plans.
8. Participate in case conferences and team meetings to discuss patient progress and develop effective treatment plans.
9. Stay current on best practices and developments in the field of social work, particularly as it relates to pediatric kidney transplant.
10. Adhere to ethical and professional standards, maintaining confidentiality and boundaries with patients and families at all times.
