Hospice Referral Coordinator

Responsibilities:

1. Coordinate and facilitate the referral process for hospice and palliative care services.

5. Maintain up-to-date knowledge of hospice and palliative care services and eligibility requirements.
6. Develop and maintain relationships with external referral sources.
7. Provide education and counseling to patients, families, and caregivers.
8. Monitor and evaluate the quality of care provided to patients.
9. Assist with the development and implementation of hospice and palliative care policies and procedures.
10. Document patient information and care in accordance with regulations and standards.
