  1. Conduct medication safety reviews and assessments to identify potential risks and prevent errors.
  2. Collaborate with other healthcare professionals to develop and implement medication safety policies and protocols.
  3. Monitor and analyze medication error reports, identify trends, and make recommendations for improvement.
  4. Provide education and training to healthcare staff on safe medication practices and the proper use of medication-related technology.
  5. Maintain up-to-date knowledge of medication safety standards and regulations.
  6. Participate in multidisciplinary teams to promote a culture of safety and quality improvement.
  7. Develop and maintain medication safety performance metrics and report findings to leadership.
  8. Investigate and respond to medication safety concerns or incidents in a timely and thorough manner.
  9. Serve as a resource for medication safety-related inquiries and provide guidance on best practices.
  10. Conduct audits and inspections to ensure compliance with medication safety policies and processes.
  11. Stay current with new medications, medication errors, and medication safety practices through continuous education and training.
  12. Collaborate with pharmacy staff to ensure proper storage, preparation, and dispensing of medications.
  13. Identify and implement strategies to reduce medication errors and improve medication safety.
  14. Assist with the development and implementation of medication safety improvement initiatives.
  15. Communicate with patients and families about medication safety concerns and provide education on safe medication practices.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Current Licensure As A Pharmacist In The State Of Ohio

  • Board Certification In Medication Safety (Bcps), Or Willingness To Obtain Within First Year Of Employment

  • Minimum Of 3 Years Of Experience In A Hospital Or Healthcare Setting, With A Focus On Medication Safety

  • Strong Understanding Of Medication Safety Best Practices, Regulatory Requirements, And Quality Improvement Methodologies
