
Clinical Pharmacist
We’re seeking an experienced, compassionate, and detail-oriented Clinical Pharmacist to join our team at Community Health Network. In this role, you will provide pharmaceutical services to our patients, while ensuring adherence to all regulatory and quality standards. The ideal candidate is knowledgeable in a wide variety of pharmaceuticals, and is passionate about helping vulnerable populations receive the care they need.The Clinical Pharmacist at Community Health Network is responsible for providing medication therapy management services and consulting with patients, physicians, and other healthcare professionals to optimize patient outcomes. Responsibilities for this role include:
- Provide medication therapy management services and consult with patients, physicians, and other healthcare professionals to optimize patient outcomes.
