Job Description

We’re seeking an experienced, compassionate, and detail-oriented Clinical Pharmacist to join our team at Community Health Network. In this role, you will provide pharmaceutical services to our patients, while ensuring adherence to all regulatory and quality standards. The ideal candidate is knowledgeable in a wide variety of pharmaceuticals, and is passionate about helping vulnerable populations receive the care they need.The Clinical Pharmacist at Community Health Network is responsible for providing medication therapy management services and consulting with patients, physicians, and other healthcare professionals to optimize patient outcomes. To be successful in this role, you should have excellent communication skills and be able to work in a fast-paced environment.The ideal candidate will possess the following qualifications:• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree, or equivalent• Current licensure as a Pharmacist in the state of Indiana• Prior experience as a Clinical Pharmacist or in a clinical setting• Knowledge of pharmaceuticals and their appropriate usage• Ability to work independently and as part of a team• Excellent communication and organizational skillsIf you’re an experienced pharmacist looking to make a positive impact on the lives of vulnerable populations, we want to hear from you. Responsibilities for this role include:

  1. Provide medication therapy management services and consult with patients, physicians, and other healthcare professionals to optimize patient outcomes.
  2. Ensure adherence to all regulatory and quality standards.
  3. Use excellent communication skills to work in a fast-paced environment.
  4. Maintain current licensure as a Pharmacist in the state of Indiana.
  5. Utilize knowledge of pharmaceuticals and their appropriate usage.
  6. Work both independently and as part of a team.
  7. Possess excellent communication and organizational skills.
  8. Monitor drug inventories and provide medication information and education to patients.
