  1. Develop and implement individualized care plans for patients with behavioral health conditions, in collaboration with healthcare providers and other professionals.
  2. Coordinate and manage the care of patients, including scheduling appointments, conducting follow-up assessments, and ensuring access to necessary resources and services.
  3. Conduct thorough assessments of patients' mental health needs and identify any potential barriers to treatment.
  4. Collaborate with internal and external stakeholders to ensure effective communication and coordination of care.
  5. Monitor and track patient progress, making necessary adjustments to care plans as needed.
  6. Provide education and support to patients and their families regarding mental health conditions, treatment options, and available resources.
  7. Stay updated on current trends and evidence-based practices in the field of behavioral health services.
  8. Maintain accurate and timely documentation of patient interactions, treatment plans, and progress.
  9. Advocate for patients and their families to ensure they receive the best possible care and services.
  10. Participate in team meetings and case conferences to discuss patient care and develop strategies for improving outcomes.
  11. Collaborate with insurance providers to ensure coverage for necessary services and treatments.
  12. Maintain confidentiality and adhere to all ethical and legal standards in the delivery of services.
  13. Act as a liaison between patients, healthcare providers, and community resources to facilitate access to appropriate care and support.
  14. Continuously assess and improve upon the effectiveness of care delivery and make recommendations for process improvements.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 3 Years Experience In Behavioral Health Care Planning And Case Management.

  • Strong Knowledge Of Mental Health Disorders And Treatment Options.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Providers, And Other Healthcare Professionals.

  • Familiarity With Insurance Coverage And Reimbursement Processes For Behavioral Health Services.
