Community Health Worker

Responsibilities:

1. Provide community-based health services and education to clients and their families.
2. Develop and maintain relationships with community partners to ensure access to health care and support services.
3. Engage with clients in a culturally sensitive manner.
4. Educate clients on health topics, such as nutrition, physical activity, and preventative care.
5. Monitor the progress of clients to ensure they are receiving the necessary services.
6. Serve as a liaison between clients and health care providers.
7. Develop and implement health education and outreach programs.
8. Identify and refer clients to appropriate resources.
9. Maintain accurate records of client contacts and services provided.
10. Provide emotional support and assistance to clients and their families.
