- Provide personalized coaching and support to individuals in achieving their health and wellness goals
- Develop and implement tailored action plans for each individual based on their specific needs and goals
- Conduct regular check-ins and follow-ups with individuals to track progress and provide ongoing support
- Educate individuals on healthy lifestyle choices and provide resources for maintaining a healthy lifestyle
- Collaborate with healthcare providers and other members of the Healthy Outcomes Team to ensure comprehensive and coordinated care for individuals
- Stay up-to-date on current health and wellness research and trends to provide the most relevant and effective coaching strategies
- Utilize motivational interviewing techniques to encourage and empower individuals to make positive changes
- Maintain accurate and organized records of all coaching interactions and progress towards health goals
- Actively participate in team meetings and trainings to continuously improve coaching skills and strategies
- Serve as a positive and professional representative of CVS Health, promoting our commitment to improving the health and well-being of our customers.
Bachelor's Degree In A Health-Related Field Such As Nutrition, Exercise Science, Or Public Health.
Certification As A Health Coach From A Reputable Organization Such As The National Board For Health And Wellness Coaching.
Experience In Health Coaching, Preferably In A Clinical Setting Or With A Diverse Population.
Strong Communication And Interpersonal Skills To Effectively Engage And Motivate Clients.
Knowledge Of Evidence-Based Health And Wellness Practices, Including Behavior Change Techniques And Healthy Lifestyle Interventions.
