Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.
Active And Unrestricted Pharmacist License In The State Of Employment.
Completion Of A Medication Therapy Management (Mtm) Certification Program.
Strong Clinical Knowledge And Experience In Medication Therapy Management, Including Assessing Patient Needs, Developing Care Plans, And Monitoring Therapy Outcomes.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Providers And Educate Patients On Medication Usage And Adherence.
