1. Provide emotional support to patients living with kidney disease.
2. Connect patients with necessary resources, such as financial assistance, housing, and transportation.
3. Advocate for the overall well-being of patients and their families.
4. Establish and maintain relationships with patients, families, and healthcare professionals.
5. Conduct thorough assessments to identify and address the psychosocial needs of patients.
6. Develop and implement individualized care plans in collaboration with a multidisciplinary team.
7. Collaborate with community organizations to expand support services for patients.
8. Offer counseling and crisis intervention services as needed.
9. Educate patients and families about kidney disease and available resources.
10. Document all interactions and interventions in a timely and accurate manner.
