1. Provide emotional support and counseling to patients and their families to help them cope with the challenges of chronic kidney disease.
2. Conduct comprehensive assessments to identify patients' social and emotional needs and develop individualized care plans.
3. Collaborate with the interdisciplinary healthcare team to coordinate and implement patient care plans.
4. Facilitate patient access to community resources and support services, such as financial assistance and transportation.
5. Advocate for patients' rights and help them navigate the healthcare system.
