- Provide comprehensive support to patients and their families, including emotional support, resources, and advocacy.
- Collaborate with interdisciplinary team to develop and implement personalized care plans for patients.
- Conduct thorough assessments of patients' social and emotional needs.
- Develop and maintain relationships with community resources and organizations to provide additional support for patients.
- Provide counseling and support to patients and families regarding healthcare decisions and end-of-life care.
- Assist patients in navigating the healthcare system and accessing necessary resources.
- Advocate for patients' rights and promote social justice within the healthcare system.
- Document all patient interactions, assessments, and care plans accurately and in a timely manner.
- Participate in team meetings and case conferences to discuss patient progress and address any concerns.
- Continuously educate oneself on social work best practices and stay updated on policies and regulations related to healthcare and social work.
- Communicate effectively with patients, families, and healthcare team members to ensure coordinated and comprehensive care.
- Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
- Support and promote DaVita Kidney Care's mission, vision, and values in all aspects of the role.
- Collaborate with other social workers and healthcare professionals to provide a supportive and cohesive work environment.
- Participate in quality improvement initiatives and assist in developing and implementing strategies for better patient outcomes.
Master's Degree In Social Work: A Minimum Of A Master's Degree In Social Work (Msw) From An Accredited University Is Required For This Position. This Qualification Ensures That The Candidate Has The Necessary Knowledge And Skills To Work In A Clinical Setting And Provide Support To Patients With Kidney Disease.
Valid Social Work License: The Candidate Must Possess A Valid Social Work License In The State Where They Will Be Working. This License Ensures That The Candidate Has Met The Necessary Requirements And Has Been Approved To Practice As A Social Worker In Their State.
Experience In Healthcare Or Related Field: Previous Experience Working In A Healthcare Setting Or With Patient Populations With Chronic Conditions Is Highly Preferred For This Position. This Experience Demonstrates The Candidate's Understanding Of The Healthcare System And Their Ability To Provide Support To Patients And Their Families.
Strong Communication And Interpersonal Skills: As A Social Worker, The Candidate Will Be Working Closely With Patients, Their Families, And A Multidisciplinary Team Of Healthcare Professionals. Therefore, Excellent Communication And Interpersonal Skills Are Essential For Building Relationships, Advocating For Patients, And Coordinating Care.
Knowledge Of Social Work Principles And Ethics: A Deep Understanding Of Social Work Principles And Ethics Is Necessary For This Position. The Candidate Must Be Able To Navigate Complex Ethical Dilemmas, Maintain Professional Boundaries, And Uphold The Values Of The Social Work Profession While Providing Care To Patients.
