Your role will be crucial in helping our patients navigate the challenges of living with kidney disease and ensuring they receive the support and resources they need to lead fulfilling lives.We are seeking a highly qualified and caring individual who holds a Master of Social Work (MSW) degree and is licensed in the state of employment. The ideal candidate will have a strong background in healthcare, exceptional communication and interpersonal skills, and a genuine passion for making a difference in the lives of others.   1. Conduct comprehensive psychosocial assessments of patients and their families to determine their needs and develop individualized care plans.
  2. Provide emotional support and counseling to patients and their families to help them cope with the challenges of living with kidney disease.
  3. Collaborate with the interdisciplinary healthcare team to coordinate and facilitate patient care, including discharge planning, referrals, and follow-up services.
  4. Connect patients and their families with community resources and support services to address their social, financial, and emotional needs.
  5. Advocate for patients' rights and ensure they receive high-quality, culturally sensitive care that meets their physical, emotional, and spiritual needs.
  6. Develop and implement intervention strategies to promote patient self-management and improve their overall quality of life.
  7. Participate in the development and implementation of quality improvement initiatives to enhance the social services provided to patients.
  8. Maintain accurate and timely documentation of patient interactions and services provided in compliance with state and federal regulations.
  9. Stay updated on current social work practices, standards, and regulations to ensure the delivery of high-quality and ethical services to patients.
  10. Serve as a liaison between patients, families, and healthcare providers to promote effective communication and collaboration.
  11. Participate in interdisciplinary team meetings, case conferences, and other meetings to discuss patient care and develop effective care plans.
  12. Provide crisis intervention and support to patients and their families during difficult situations, such as end-of-life care.
  13. Collaborate with insurance companies, government agencies, and other organizations to obtain coverage for necessary services and resources for patients.
  14. Participate in the onboarding and training of new social workers and other team members to ensure the delivery of consistent and high-quality services.
  15. Continuously evaluate and monitor the effectiveness of social services provided and make necessary adjustments to ensure optimal patient outcomes.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Institution: A Solid Educational Foundation Is Essential For A Social Worker At Davita Kidney Care As It Provides The Necessary Knowledge And Skills To Effectively Work With Patients And Their Families.

  • Relevant Experience In A Healthcare Setting: Previous Experience In A Healthcare Setting, Particularly In The Field Of Chronic Kidney Disease, Is Highly Desirable For This Position.
