1. Advocate for patients' rights and well-being throughout their healthcare journey.
2. Serve as a liaison between patients, their families, and healthcare providers to ensure effective communication and coordination of care.
3. Collaborate with healthcare teams to develop personalized care plans and ensure they are followed.
4. Actively listen to patients' concerns and address any issues or complaints promptly and effectively.
