As a Hospice Aide, you will play a crucial role in providing physical, emotional, and spiritual support to our patients and their families in their homes or in our hospice facility. - Provide physical care and assistance to patients with daily tasks such as bathing, dressing, and grooming.
- Monitor and report any changes in patient's condition to the hospice team.
- Assist with medication reminders and administration.
- Provide emotional support and companionship to patients and their families.
- Help patients maintain a clean and safe environment in their homes or in the hospice facility.
- Assist with light household chores, meal preparation, and grocery shopping.
- Follow the care plan outlined by the hospice team and communicate any concerns or observations.
- Maintain accurate and timely documentation of care provided.
- Collaborate with other healthcare professionals involved in the patient's care.
- Attend ongoing training and education to maintain knowledge and skills in hospice care.
- Respect and maintain patient confidentiality at all times.
- Uphold the mission and values of Deaconess Health System in all interactions with patients, families, and colleagues.
High School Diploma Or Equivalent: A Minimum Educational Requirement For A Hospice Aide At Deaconess Health System Is A High School Diploma Or Equivalent. This Ensures That The Candidate Has A Basic Level Of Education And Understanding To Perform The Job Duties Effectively.
Certified Nursing Assistant (Cna) Certification: Deaconess Health System Requires All Hospice Aides To Hold A Valid Cna Certification. This Certification Demonstrates That The Candidate Has Received Proper Training And Is Competent In Providing Care To Patients.
Hospice Experience: Candidates With Prior Experience In Hospice Care Will Be Preferred For This Role. This Experience Shows That The Candidate Is Familiar With The Unique Needs Of Hospice Patients And Is Comfortable Working In A Hospice Environment.
