Job Description

  1. Provide compassionate and quality care to hospice patients during their final days.
  2. Support patients and their families by providing comfort and emotional support.
  3. Administer medications and treatments as prescribed by the hospice physician.
  4. Monitor and assess patients' physical and emotional well-being.
  5. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  6. Educate patients and families about the hospice process and provide information on available resources and support.
  7. Document all patient care activities and maintain accurate patient records.
  8. Participate in regular team meetings to discuss patient care and make care plan adjustments as needed.
  9. Serve as a liaison between patients, their families, and the hospice team.
  10. Provide emotional support and counseling to patients and their families during the grieving process.
  11. Communicate effectively with physicians, healthcare providers, and other team members to ensure coordinated and comprehensive care.
  12. Remain current on hospice care best practices and regulations.
  13. Adhere to all safety and infection control protocols.
  14. Maintain confidentiality of patient information.
  15. Uphold the mission, values, and standards of Deaconess Health System.

Job Qualifications
  • Active Rn License In The State Of Employment: The Hospice Nurse Must Hold A Valid And Current Rn License In The State Where They Will Be Practicing. This Ensures That They Meet All Necessary Education And Training Requirements To Provide Quality Care To Patients.

  • Minimum Of 2 Years Of Nursing Experience, Preferably In Hospice: Deaconess Health System Requires A Minimum Of 2 Years Of Nursing Experience, With A Preference For Candidates Who Have Worked In A Hospice Setting. This Ensures That The Candidate Has A Strong Foundation In Nursing And Is Familiar With The Unique Needs Of Hospice Patients.

  • Bls Certification: Basic Life Support (Bls) Certification Is A Requirement For All Nurses At Deaconess Health System. This Ensures That The Candidate Is Trained In Life-Saving Techniques And Can Respond Quickly In Case Of An Emergency.

  • Excellent Communication And Interpersonal Skills: As A Hospice Nurse, The Candidate Will Be Working Closely With Patients, Families, And Other Healthcare Professionals. Strong Communication And Interpersonal Skills Are Essential For Building Relationships And Providing Compassionate Care To Patients And Their Families.

  • Compassionate And Empathetic Nature: Deaconess Health System Is Looking For Candidates Who Possess A Compassionate And Empathetic Nature. Hospice Nurses Must Be Able To Provide Emotional Support To Patients And Their Families During A Difficult Time, And Having A Caring And Empathetic Nature Is Crucial For This Role.
