Bachelor's Degree In Social Work, Nursing, Public Health, Or A Related Field.
Minimum Of 2-3 Years Of Experience In Care Coordination Or Case Management.
Knowledge Of Health Care Systems And Resources, Including Insurance Coverage And Government Programs.
Excellent Communication And Interpersonal Skills, With The Ability To Work With Diverse Populations.
Strong Organizational And Problem-Solving Skills, With The Ability To Prioritize And Manage Multiple Tasks In A Fast-Paced Environment.
