- Dispensing Medications: The Clinical Pharmacist is responsible for accurately and safely dispensing medications to patients in accordance with state and federal regulations.
- Medication Therapy Management: Conduct comprehensive reviews of patient medication histories and provide recommendations for optimizing medication therapy to improve patient outcomes.
- Patient Education: Educate patients on proper medication usage, potential side effects, and drug interactions to ensure safe and effective use of medications.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans for patients.
- Medication Safety: Monitor for potential drug interactions, allergies, and other adverse reactions to ensure patient safety.
- Quality Assurance: Perform routine audits and quality checks to maintain compliance with state and federal regulations and standards of practice.
- Patient Counseling: Provide counseling and support to patients on proper medication adherence, lifestyle modifications, and disease management.
- Drug Utilization Review: Conduct regular reviews of medication orders to ensure appropriate dosing, duration, and therapy outcomes.
- Continuous Learning: Keep up-to-date with current developments and advancements in the field of pharmacy and apply this knowledge to improve patient care.
- Documentation and Record-Keeping: Maintain accurate and complete patient records, including medication profiles, drug interactions, and medication therapy plans.
- Teamwork and Collaboration: Work collaboratively with other healthcare professionals to promote a multidisciplinary approach to patient care.
- Patient Advocacy: Serve as an advocate for patients by addressing their concerns and promoting their rights to access safe and effective medications.
- Adherence to Policies and Procedures: Adhere to all company policies, procedures, and protocols to ensure the delivery of high-quality and consistent patient care.
- Professional Development: Participate in ongoing training and professional development opportunities to enhance knowledge and skills and maintain professional licensure.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Valid State Pharmacist License And In Good Standing With The State Board Of Pharmacy.
At Least 2-3 Years Of Experience In A Clinical Setting, Preferably In A Healthcare Facility Or Managed Care Organization.
Strong Knowledge Of Pharmacotherapy And Medication Management, Including Drug Interactions, Adverse Effects, And Dosing Regimens.
Excellent Communication, Critical Thinking, And Problem-Solving Skills, With The Ability To Work Collaboratively With Healthcare Teams And Patients.
