Job Description

At Elevance Health, we are seeking a compassionate and knowledgeable Pharmacist to join our team. Our Pharmacists are integral members of a multidisciplinary team, providing care to our patients and their families. This position requires expertise in medication management, medication safety, inpatient pharmacy operations, and patient education. The successful candidate must have a Pharm.D. degree from an accredited college or university and at least two years of experience in a hospital setting.The Pharmacist will have an opportunity to make a meaningful impact on the lives of our patients and their families. The team-oriented Pharmacist must possess excellent communication and interpersonal skills, as well as the ability to work independently and collaboratively. Responsibilities:

  1. Provide medication management, medication safety, and inpatient pharmacy operations services to patients.
  2. Educate patients and their families about medication safety and usage.
  3. Develop and maintain professional relationships with other healthcare providers and members of the multidisciplinary team.
  4. Ensure compliance with applicable regulations and standards.
  5. Participate in continuing education and professional development activities.
  6. Monitor patient medication profiles and ensure accuracy and completeness.
  7. Develop and implement medication protocols and policies.
  8. Prepare and dispense medications in accordance with established procedures.
  9. Provide timely and accurate information to healthcare providers and other members of the multidisciplinary team.
  10. Respond to inquiries from patients, families, and healthcare providers.
