Responsibilities:

1. Assist Home Therapy nurses with patient care tasks.
2. Communicate effectively with Home Therapy patients.
3. Monitor patient progress and update patient records.
4. Work collaboratively with other members of the Home Therapy team.
5. Respond to patient inquiries in a timely and professional manner.
6. Ensure that patient needs are met according to Fresenius Medical Care standards.
7. Ensure patient safety and adhere to all relevant regulations.
8. Conduct follow-up visits with patients and document results.
9. Monitor Home Therapy supplies and equipment and order replacements as needed.
10. Assist with patient transportation and other needs as required.
