As an MDS Coordinator, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for our residents, ensuring accurate and timely documentation.   1. Coordinate and complete Minimum Data Set (MDS) assessments for residents in a timely and accurate manner.
  2. Ensure all MDS documentation is in compliance with guidelines and regulations.
  3. Utilize strong clinical skills to accurately assess and document resident needs.
  4. Collaborate with interdisciplinary team to develop and implement individualized care plans.
  5. Communicate effectively with team members, residents, and families to ensure continuity of care.
  6. Maintain thorough understanding of MDS guidelines and regulations, staying up-to-date on any changes.
  7. Review and analyze resident data to identify trends and make recommendations for care improvements.
  8. Participate in quality improvement initiatives related to MDS and resident care.
  9. Provide education and training to staff on MDS and related regulations.
  10. Serve as a resource for staff, residents, and families regarding MDS and resident care.
  11. Adhere to all company policies and procedures, as well as state and federal regulations.
  12. Maintain confidentiality of resident information and comply with HIPAA regulations.
  13. Actively participate in meetings and contribute to the overall success of the team.
  14. Continuously seek opportunities for professional development and growth.
  15. Demonstrate professionalism, compassion, and empathy in all interactions with residents and their families.

Job Qualifications
  • Current Lpn License: The Candidate Must Hold A Valid And Current Lpn License In The State Where The Facility Is Located.

  • Minimum Of 2 Years Of Experience In Long-Term Care: The Ideal Candidate Should Have At Least 2 Years Of Experience Working As A Lpn In A Long-Term Care Setting.

  • Knowledge Of Mds And Medicare Guidelines: The Mds Coordinator Is Responsible For Completing And Submitting Accurate Mds Assessments And Coordinating With Medicare Reimbursement Requirements, So A Strong Understanding Of These Guidelines Is Essential.

  • Strong Communication And Interpersonal Skills: The Mds Coordinator Will Work Closely With A Variety Of Team Members, Including Physicians, Nurses, And Other Healthcare Professionals, So Excellent Communication And Interpersonal Skills Are Necessary.

  • Detail-Oriented And Organized: The Mds Coordinator Must Be Able To Review And Analyze Complex Medical Records And Accurately Complete Mds Assessments. Attention To Detail And Strong Organizational Skills Are Critical For Success In This Role.
