Health Outcomes Pharmacist

You must have a Pharmacy degree from an accredited college, as well as a license to practice pharmacy in the state of Pennsylvania. Additionally, knowledge of electronic medical records and/or data analytics is a plus.You must be a highly organized and detail-oriented professional with excellent communication and problem-solving skills. You should be a self-starter with the ability to think critically and work independently, as well as the capability to work collaboratively in a dynamic and fast-paced environment. Above all, you must have a commitment to providing the highest quality of care to our members.

Responsibilities of a Health Outcomes Pharmacist:

1. Manage drug therapy programs and clinical initiatives to ensure the highest quality of care to members.
2. Develop policies and procedures to ensure compliance with state and federal regulations.
3. Utilize electronic medical records and data analytics to identify trends and improve outcomes.
4. Monitor patient progress and provide feedback to providers.
5. Collaborate with other healthcare professionals to optimize patient care.
6. Educate patients and families on proper medication use.
7. Participate in research projects to improve the health outcomes of patients.
8. Maintain knowledge of current pharmacy practices and developments.
9. Provide consultation services to healthcare providers.
10. Prepare and dispense medications as needed.
