  11. Participate in ongoing education and training to stay current on healthcare advancements and best practices.
  12. Uphold the values and mission of Highmark Health in all interactions with patients, colleagues, and the community.
  13. Collaborate with community resources and organizations to provide additional support and resources for patients.
  14. Serve as a mentor and resource for other healthcare professionals, sharing knowledge and expertise in patient navigation.
  15. Maintain strict confidentiality and adhere to ethical standards in all aspects of the job.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.

  • Active And Unrestricted Rn License In The State Of Employment.

  • Minimum Of 3-5 Years Of Clinical Nursing Experience, Preferably In A Specialized Area Such As Oncology Or Chronic Disease Management.

  • Certification In Case Management Or Patient Navigation (E.g. Ccm, Onn) Is Preferred.

  • Strong Communication, Organizational, And Critical Thinking Skills, With The Ability To Work Independently And Collaboratively With A Multidisciplinary Team.
